However, current neoantigen prediction pipelines often lack direct integration of mass spectrometry–based immunopeptidomics, limiting the discovery of antigens from canonical and non-canonical sources. We developed NeoDisc, a rapid, modular clinical proteogenomic pipeline that integrates genomics, transcriptomics, and immunopeptidomics with advanced in silico tools to identify and prioritize tumor-specific antigens, including neoantigens, viral antigens, and non-canonical peptides. Through multi-omics integration, NeoDisc enables the discovery of peptides arising from alternative reading frames, aberrant translation events, and other noncanonical sources, while also revealing defects in antigen presentation machinery that shape the tumor antigenic landscape. NeoDisc is currently being applied in clinical Phase I trials to support real-time, clinically actionable neoantigen discovery for personalized cancer immunotherapy.
